Embraer ERJ-135 —
2003 - a Southwest B737 about to execute a go-around after the Chautauqua
(Delta) E135 failed to rotate in time on former runway 13.

In this picture the go around decision is loong overdue, 500 - 700 ft AGL would have been more appropriate. Were these guys napping or just memorized and complacent? I'm suspecious that it's not a photo shopped image.
Mark, your suspicions that "it's not a photo shopped image" is correct, it is not Photoshopped. I've been taking photos of aircraft for 52 years and have never added another aircraft to an image except for an airline customer request to do for their corporate use.
Reports must have been filed and recommendations as a result or worst. This should not have happened . The worst lost of life occured while aircraft were on the ground or there about . Tenerife .
The tower sure blew this one; also, over reliance on computers and not enough time outside the cockpit during landing.
What happen to the 3 minute separation rules??
They blow past the 3 min/miles rule at DCA all the time. I can't tell you how many times I've seen a plane turning over Gravelly point as the plane on 19 begins to rotate. Right or wrong, in VFR conditions you can get away with it I suppose.
